Making it convenient for members to connect to and navigate the health care system

The Health Care on the Go mobile clinic connects doctors and nurses with members who face barriers getting to the doctor.

The Health Plan continues to anticipate its customers' needs and differentiate itself through the variety of services it offers to members to ensure an exceptional experience.

The Health Care on the Go mobile clinic is connecting doctors and nurses with Health Plan members who face barriers getting to the doctor. The mobile clinic provides preventive care for at-risk members who often forgo medical care and who are not close to a physician's office. The mobile clinic features two exam rooms, a wheelchair lift and fully accessible bathroom, as well as the appropriate refrigeration needed to store medicines and lab specimens. Highmark care coordinators reach out to members who have not had preventive exams recently and arrange for a wellness visit. The clinic not only offers preventative exams, but also offers biometric screenings, immunizations and vaccines, smoking cessation counseling, and more.

"We are targeting our most at-risk members — ones who, for one reason or another, we know are not getting the care they need," said Barbara Gray, RN, senior vice president of clinical services at Highmark Inc. "Making it convenient for our members to connect to and navigate the health care system can change lives."

Through its Meet Dr. Right program, the Health Plan members have the opportunity to meet physicians in person prior to choosing one for their care. These scheduled events introduce members to doctors of various specialties prior to making an appointment. Ensuring a doctor is a good match for Health Plan members is important and can improve outcomes, enhance member satisfaction, and reduce costs.

"Finding a doctor that you are comfortable with can be challenging," said Dr. Charles DeShazer, vice president and executive medical director of clinical strategy for the Health Plan. "We provide these free Meet Dr. Right events to help individuals meet and talk with a doctor before they make an appointment. This helps ensure the patient experience will be a good one."

For members dealing with cancer, the Health Plan offers a dedicated team of case managers to help them through their cancer treatment journey. These cancer outreach nurses are there to provide members with the support and resources they need to "connect the dots" of their cancer care. Whether it's information on staying healthy during cancer treatment or help navigating the complexities of the health care system, members receive the guidance, education, and support they need during one of the hardest times of their lives.
