Headquartered in Pittsburgh, Pennsylvania, Helion, an independent company that provides post-acute network management services for Highmark Inc. and its affiliated health plans, delivers solutions to health plan payers to advance health and healing in the home by transforming the network of providers and nurses to optimize outcomes, provide better value, and enhance the customer experience.

The foundation of Helion's strategy is centered on enabling patients to receive care in the least restrictive and most desirable environment, such as the home.

Helion's vision is a future where the home is embraced as the keystone of the care continuum. Its mission is to empower patients, providers, and payers to advance health and healing in the home through informed collaboration.

In 2024, Helion provided services in multiple states — which included collaborating with over 5,500 post-acute and outpatient therapy partners. Helion products and services include a suite of data- and analytic-driven services that bring evidence and insights to the home and community care network. Additionally, it offers a network optimization system that provides a path to true value-based care and optimized network performance. These two key components work together to unlock the potential of home and community care for payers and providers so they can deliver prevention, condition management, and healing — helping providers elevate what they do and positively impact the entire care continuum.

Building a better future through clinical programs, collaborative partnerships, and community initiatives

At the heart of Helion's mission is a commitment to clinical innovation, home-centered care, and community.

Helion aims to continuously advance its mission to build a better future of health care. It begins with clinical initiatives that enrich the member experience, leading to better outcomes; collaborative partnerships that evolve solutions for industry challenges; shifting mindset to envision home as the center of care; and championing community efforts that support members where they live.

Through a collaborative effort with Allegheny Health Network (AHN) and Helion's network of skilled nursing facilities (SNFs), what began as a pilot program during the pandemic has evolved into a robust solution for transitioning patients and mitigating staff or bed availability challenges. Leveraging existing technology and teams, AHN contractually reserves beds from select SNFs near their hospitals, facilitating timelier transitions to that next level of care. This enables members to begin therapy without delay. Regular collaboration with hospital and facility leaders, through real-time quality reviews, has fostered a true continuum of care.

Helion's strategic partnerships enable the Care at Home program, an innovative model that supports the delivery of facility-level care in the comfort of home. This includes Hospital at Home, which offers inpatient hospital-level care, and Advanced Recovery Care at Home for post-acute level care. Both aim to reduce readmissions and empower patients and families to redefine care delivery beyond hospital walls.

While much of Helion's day-to-day work focuses on transforming care along the continuum, addressing social determinants of health (SDOH) is also part of the mission. Members' needs often extend beyond the clinical, including food, clothing, housing, or environmental insecurities. Through connections with nonprofit organizations, Helion associates regularly devote time and resources to social and community programs that support whole person health.

Through innovative partnerships, home-centered care models, and a commitment to addressing SDOH, Helion is creating a more patient-centric, efficient, and compassionate care experience — one initiative, one partnership, and one community member at a time.

Over 90% of home health agencies and over 80% of skilled nursing facilities in the networks Helion serves are in a value-based arrangement. 

Approximately 87% of home health episodes and 82% of skilled nursing facility episodes are driven to high-performing networks.
