Highmark Health’s purpose is to do something remarkable for the people and communities we serve. By connecting payer, provider, technology solution partners, community organizations, and others, we are taking on systemic challenges that the health industry has struggled with for decades.
Our Living Health model leverages our comprehensive data, insights, relationships, and influence to better connect and support each person’s whole health. That’s our path to improving health experiences and outcomes as well as addressing the cost crisis affecting everyone who pays for care: individuals and families, employers, and governments.
2025 was difficult for many organizations in health care, including ours. Headwinds included a third straight year of near double-digit increases in combined medical and pharmacy cost trends, government policy changes, and ongoing financial pressures on hospitals. We reported operating losses for both Highmark health plans and our enterprise.
However, there is much more to our story for 2025. Despite the challenging health care landscape, Highmark Health recorded a fifth consecutive year of revenue growth, from $29.4 billion in 2024 to $32.4 billion in 2025. We saw a stunning $237 million margin turnaround at Allegheny Health Network (AHN)* and record revenue from United Concordia Dental.* Most importantly, our resilience, diversified structure, and fundamentally strong balance sheet enabled us to invest $569 million to build our transformational Living Health model, improve our communities, and advance our mission to create a remarkable health experience, freeing people to be their best.
As part of our commitment to expand access to high-quality care and affordability, we also announced two affiliations: Blue Cross Blue Shield of Kansas City (1 million+ members), and, pending approval, Heritage Valley Health System (two hospitals, 600 physicians) in western Pennsylvania.
Given the challenges and pace of change in 2025, we are prouder than ever to share a few highlights of what our team members achieved to benefit those we serve.
This report details Highmark Health's efforts to enhance health experiences and outcomes, deepen community engagement, leverage advanced technology, drive operational excellence, and ensure sustainable growth for the people and communities we serve.

Highmark health plans, comprised of Highmark Inc. and its affiliated health plans, continued to improve the member experience and health outcomes in 2025. To start, two more curated digital offerings were added to the suite of more than 35 Living Health solutions:
Highmark health plans use insights from data across digital solutions and the My Highmark app to deliver a more personalized and engaging member experience, and to identify opportunities to improve the products and services on the member’s behalf. For example, enhancements in the “Find & Schedule Care” tool within My Highmark facilitated over 837,000 searches, which helped health plan members find and access the right providers faster. Qualitative and experiential insights can be just as valuable: in 2025, a newly launched program enabled health plan members to share their own remarkable health stories.
Highmark health plans’ Medicare Advantage offerings provide another example of the commitment to improving health outcomes and experiences for members. Of the top 10 largest Medicare Advantage carriers by enrollment, Highmark health plans have the second highest percentage (95%) of members enrolled in a plan that CMS rated 4 Stars or higher.
As it has for more than a decade, AHN* continued to expand care access into the communities served while delivering remarkable patient experiences, quality, and outcomes. 2025 highlights include:

Regardless of industry ups and downs and societal changes, Highmark Health’s commitment to its communities is unwavering and involves every part of the enterprise.
In June 2025, Highmark Health officially launched the Social Health Center of Excellence, an expansion and strategic centralization of social determinants of health (SDOH) efforts that cross the enterprise, including Highmark health plans and AHN.* The Living Health model’s whole-health approach means that the organization develops social health capabilities with the same rigor that goes into developing physical and mental health capabilities. The SDOH team is leading the industry in this area, including publishing research in JAMA. Through expanded multiyear “food as medicine” programs, Highmark Health helped more people gain access to nutritious food. The impact of these programs has received national recognition, including being featured by the Blue Cross Blue Shield Association in 2025.
AHN provided $763 million in charitable and uncompensated care in 2025. AHN deepened its commitment to accessible care by actively engaging the community. In 2025, the AHN Cancer Institute conducted 1,456 no-cost cancer screenings and the community impact team provided over 6,200 additional screenings, including echocardiograms, biometrics, and COPD. Additionally, an ongoing collaboration between the nationally renowned First Steps and Beyond Program and AHN Women’s Health Institute helped more than 30,000 people through maternal health initiatives, including a Food Rx program, doula care, and mobile resources.
In 2025, Highmark Health and its affiliates and subsidiaries invested over $56 million in corporate and foundation giving in Pennsylvania, Delaware, West Virginia, and New York. Highmark awarded a $1 million grant to Achieva, which launched a program to transition children with complex medical needs from institutions to their homes. This program provides comprehensive support and strengthens families, reflecting a deep commitment to community-based solutions that enhance well-being and reduce long-term health care costs. Highmark Wholecare’s Farm-to-Family program distributed nearly 250,000 pounds of free, fresh produce to more than 11,000 families from its start in 2024 to the end of 2025.

The fast-evolving capabilities and uses of artificial intelligence dominated media, business conferences, and workplace discussions during 2025. But long before the generative AI surge began, Highmark Health was investing in and leveraging AI, predictive analytics, and other advanced data technologies to provide people with a simpler, more personalized, and proactive system for health, coverage, and care. Highmark Health’s data science, advanced analytics, and AI experts have gained national attention, including for building the Living Health Insight Platform and other technological innovations as part of a long-term partnership with Google Cloud.
Nonetheless, 2025 stands out as an exceptional year. Internally, the enterprise’s secure, dedicated generative AI platform, Sidekick, and other AI-enhanced tools have been widely embraced as part of transforming how the organization works and collaborates. enGen*, the health technology company, earned the “Best Core Administrative Processing System” award in the ninth annual MedTech Breakthrough Awards program, with specific recognition for its AI Cognitive Engine (ACE). enGen innovations like that have been improving efficiency, reducing costs, and supporting member engagement for Highmark health plans and others for more than a decade.

Highmark Health is also focused on ensuring that the people it serves get the full benefit of new technology. A great example is a collaboration between Helion*, which focuses on home and community care, and the enterprise research team. The S.U.P.P.O.R.T. for Seniors program, funded by a grant from the Henry L. Hillman Foundation, addresses barriers to seniors’ use of health-related technology and digital solutions. Community health workers collaborate with home health nurses to offer personalized, in-home support for platforms like My Highmark, MyChart, and Living Health digital solutions, complementing existing care plans.
Looking ahead, Highmark Health is committed to remaining on the leading edge of leveraging advanced technology to keep individuals engaged in their health, help clinicians diagnose and treat, get administrative tasks done faster and more efficiently, collaborate more effectively across a person’s health ecosystem, understand and manage risk, and much more.

AI-driven platforms like Sidekick and Abridge have accelerated ongoing enterprise-wide efforts to improve efficiency. Altogether, Highmark Health estimates savings of over $240 million through improved administrative efficiency over the past three years. Optimizing workforce structure to meet evolving needs, this approach focuses on rethinking how teams work; reimagining processes and roles; and adjusting hiring, training and development to prioritize skill sets that support the mission. For example, Highmark health plans’ Product & Health Experience team — central to building and advancing the Living Health model — reported a 75% reduction in cost-per-insurance product and a 33% faster speed to build. Work like that is an essential part of the enterprise commitment to making health care more affordable.
At AHN*, multiple factors contributed to a $237 million turnaround in 2025, including higher patient volumes, expanded clinical and specialty pharmacy services, and operational excellence driven by the onboarding of new leadership in 2025, with president Mark Sevco at the helm. His leadership drove efficiency, reducing reliance on staffing agencies and optimizing revenue cycles, all while fostering a unified team culture. The commitment to impactful operations also fueled environmental stewardship. Key initiatives that focused on energy efficiency, waste reduction, and reduction of anesthetic gases contributed to a healthier environment and bottom-line improvements. Dedication to operational efficiency and environmental stewardship earned AHN an NRG Excellence in Energy Award for Sustainability, making AHN the only health care provider nationwide to be recognized in 2025.
HM Insurance Group* provides another example of how operational efficiency can directly benefit the people we serve. Combining technology and operational improvements, HM Insurance Group paid 99.98% of claims in less than six days during 2025. That’s significantly better than the industry benchmark — giving policyholders confidence that catastrophic health-related claims will be paid quickly and accurately.

The new affiliations with Blue Cross Blue Shield of Kansas City and, pending approval, Heritage Valley Health System will advance the long-term strategy to scale Living Health so Highmark Health can improve health experiences, outcomes, and affordability for more individuals and communities. Such growth also strengthens the organization’s sustainability and industry influence, helping more people in more ways well into the future. Other growth highlights for 2025 align with the same core belief: What’s best for the people Highmark Health serves will be what’s best for its business.
Turning to growth that impacts the future of clinical care, AHN* Schools of Nursing recorded their largest enrollment ever in 2025, 44% higher than 2023. The Strategic Social and Workforce Programs team continued to develop its Young Scholars Program, a national model that cultivates future health care leaders starting with an engaging summer intensive for middle and high school students who are passionate about math and science. Now in its second year, the program supports two cohorts of 19 students with stipends, transportation assistance, and other guidance.
